Guardian information: name, email address, phone number, and preferred contact method.

Any additional information shared in the open-ended notes field.

This form is intended to be completed by a parent or legal guardian on the student's behalf.

To respond to your tutoring inquiry and follow up about scheduling; to match your student with appropriate tutoring support; to maintain a record of prospective and current clients; and to contact you using your preferred contact method.

We do not sell your information or use it for advertising.

Submissions are stored in a secure, access-controlled database. Data is transmitted using industry-standard encryption (HTTPS). Access is limited to McGrath Tutoring LLC.

Information is retained as long as reasonably necessary to respond to the inquiry, and for the duration of the tutoring relationship plus a reasonable record-keeping period. Deletion may be requested at any time.

Third-party providers (email delivery, hosting) may be used solely to operate the site and respond to inquiries — never for their own purposes.

You may request a copy of the information we have on file, request correction or deletion of that information, or withdraw consent for future contact, by reaching out using the contact information below.
